Assuming you are a foreign student studying in the USA, if there's anything you don't understand about this very important document you should take it immediately to the Office of International Student Services, or whoever deals with international students at your college, and ask them to help you with it. The document is telling you what you need to do if you want to leave the USA for a while and come back to your college, and if that's what you're planning to do, don't waste another day but get down there quickly and ask them to explain exactly what you need to do. It's their job to help people in your situation.
